Statistically, Tuesday is the most dangerous day of the week! - Potential Incidents, Near Misses and
accidents are all most likely to occur on a Tuesday. PI’s and NM’s are least likely to be identified on a

Friday, but accidents are least likely on a Monday.

Root Cause – The root cause of all PI’s, NM’s and incidents are analysed and recorded. The most
common being improper motivation and failure to be aware, there are both personal factors, meaning
they relate to the individual or the safety culture.

So what have we done about these incidents?

Where practicable we try to remove the hazard as and when we identify it.
We have been able to eliminate the hazard in 40% of all the incidents at the time of discovery. A
further 3% were eliminated shortly thereafter.
Where we were unable to eliminate the accident, we were able to reduce the risk in a further 17% of
cases.
