3. Vehicle registration no – give the registration number of your vehicle.
You must sign and date the section if you are transporting the waste.
Part D – Consignor’s certificate
The consignor is the person that generated the waste, this will be you. You must enter your name and
sign and date where indicated.
Part E – Consignee’s certificate
This section will be completed by the consignee, this is the person who received the waste, for lamps
this will be the CEF representative.
The consignee will confirm the details of the waste received, the quantity and how they are dealing
with the waste (Recovery / Disposal / Reject / Storage for Recovery etc), the date, and vehicle
registration number.
They will then sign and date the certificate and enter their waste management license number /
exemption number.
Please ensure this section is completed and is readable on the duplicates.
After completing sections A-D of the consignment note, you must transport the waste to the location you
have stated in Part A4 and give the form to the recipient of the waste, they must complete section E and
then you should keep a copy and give a copy to them.
You should remember that the consignment notes are legal documents, and failure to complete them or
falsification of them can result in legal action against you and the company.
