2. Removed from, this is the site name and address from where the waste is being removed.
3. Premises code, this is the registered premises code of the site. If the site is exempt, enter “n/a”. A
list of premises codes for each site will be issued when received. If you do not know the site’s
registration code contact the office.
4. Waste taken to, this is the name and address of the licensed company or transfer station that you are
taking the waste to. For lamps, this will be the address of the CEF branch that you are taking the
lamps to.
Part B – Description of the waste
1. The process giving rise to the waste, two options are given; “Sign Maintenance” and “Sign
Removal”, delete whichever is not applicable, or enter the appropriate reason in “Other”.
2. SIC code, this is a code for signage, painting and similar works and will always be 45.34.
3. Waste details, this table gives details of the hazardous waste as detailed below:
Description – a brief description of the waste, such as “fluorescent tubes” or “paint cans”.
EWC code – this is the code number of the waste as described above.
Quantity – this is the weight of the waste (or reasonable estimate).
Component – this is the hazardous component of the waste, for fluorescent tubes this is Mercury,
for other waste this should be obtained from your COSHH sheets.
Concentration – this is the amount of the component within the waste. For fluorescent tubes this
is 0.01351%, for other substances this will be identified on the COSHH sheets.
Physical form – this will be the form of the substance – solid/liquid/gas/powder/sludge/mixed.
Hazard code – this is the code stating the hazardous properties of the goods. A list of all the
codes is attached. For fluorescent tubes this will be H6 (toxic) and H14 (ecotoxic).
Container – this is the quantity and size of the waste. E.g. “5 boxes of 25” or “2no 5ltr tins”.
The form gives room for two entries, one of which is already completed with the details of
fluorescent tubes. Where fluorescent tubes are being transported, only the weight and container size
needs to be entered. When transporting other hazardous waste, delete the fluorescent tubes entry
and enter the details of the waste in the second row.
The second section of section B3 of the report is for use when transporting “Dangerous for
Carriage” goods and is not normally relevant for us. The last column is the Special handling
requirements, enter any special requirements such as “fragile”.
Part C – Carrier’s certificate
This section must be completed by the company that will transport the waste. Where more than one
carrier is to be used, (for instance you give the lamps to another crew to take to CEF) then a separate
schedule of carriers must be completed. If you are transporting the waste, then you must complete the
section.
1. Carrier name – this is your name.
2. Carrier registration no – this is the number by which Xmo Strata is registered with the Environment
Agency as a waste carrier and is already entered on the form.
