Near Miss Alert

Please see enclosed self-explanatory bulletin that was shared with us by AECOM / Global Alliance.

The first incident described in this bulletin is particularly pertinent to us and is similar to a recent
incident that occurred to one of our employees in October 2012, when water splashed into his face and
got into his eyes despite him wearing safety glasses. He later had to attend hospital to have a foreign
body removed from his eye.

Always carry out a Last Minute Risk Assessment (LMRA) to ensure all the risks are controlled and the
type of PPE being worn is appropriate.

We are all a product of learning. Every skill we have; everything we know; everything we believe and everything
we do has been learned throughout the course of our lives. Some of us learn best by studying graphs and
charts, some through visual presentations, and some by reading detailed text books. It is important that we
receive information in a format that works for us as individuals! That’s why at Xmo Strata we use a variety of
