Problems during  MID installation procedure

           This group covers issues such as insuffcient daylight to see clearly during installation, chemical anchor
           bolts failing during installation of adaptor plate, and MID contacting  the canopy during a lift.
           Problems with tools / spanners

           On  three  occasions  crews had attempted to improvise
           and  use  an  unsuitable  tool to  loosen / tighten the nuts
           instead of getting a suitable sized spanner.  In this case
           it resulted in the tool cracking.








           Problems during  MID removal procedure

           On two occasions it was found that the safe system of work that
           had been written following the survey was not practicable when
           the works were to start.  In both cases the LMRA identified the
           problem  the  work  was  stopped  and  an  alternative  system  of
           removal produced.

           All of our MID installers attend a comprehensive 4 day NOPRS
           (National Plant Operators Regestration Scheme) course before
           being accredited to do this work and the number of PI/NM’s
           reported indicates that we are vigilant and alert, but this still
           remains a very high risk for us and adopting ongoing  Last
           Minute Risk Assessments is essential, given that many  of the
           hazards are not immediaterly evident when you complete the
           clearance certificate before starting work.



           We are all a product of learning. Every skill we have; everything we know; everything we believe and everything
           we do has been learned throughout the course of our lives. Some of us learn best by studying graphs and
           charts, some through visual presentations, and some by reading detailed text books. It is important that we
           receive information in a format that works for us as individuals! That’s why at Xmo Strata we use a variety of
