Please see enclosed bulletin regarding a trip injury, shared with us by BP.
The forks of this truck are a good example of a potential hazard that could be picked up using the LMRA checklist
on page 2 of the Xmo Strata clearance certificate. Highlighting this as a hazard and then agreeing a way of
eliminating or reducing the risk of the trip hazard with the site manager is a great way to make your LMRA site
specific.
It is easy to miss trip hazards from kerb stones, gutters, or drain covers etc when carrying out a LMRA, they are so
common everywhere we go that they don’t register as a hazard. But the fact is that they are a common cause of
injuries, especially when you are concentrating on the “big” hazards.
When carrying out the LRMA, look at the risk categories on your LMRA card, consider each carefully, you will
always find some on site but don’t dismiss them because they are always there – the more common a hazard the more
likely it is to injure you.
The latest HSE statistics show that 1 fatality, 7828 major injuries, and 12466 seven-day injuries occurred due to trips,
slips and falls on the same level in the UK in 2015/16.
