              Exempt sites with no Premises code:
                 Use EXE as the first three characters.
                 Use the first three characters of the waste collection company  for the next three characters.
                 Use the three first characters of the clients name followed by a two digit unique number.

              e.g.  EXEXYZ/SHE01

              It is important that we use the same numbering systems as the client to ensure that the numbers are
              unique – this is a legal requirement.

         2. This is the site name and address from where the waste is being removed.

         3. Premises code, this is the same code that is used for the first part of the consignment note code.  A list
         of  premises  codes  will  be  issued  made  available  when  received.    Until  this  is  received,  the  number
         EXEXXX.

         4.  The  waste  will  be  taken  to  –  this  is  the  name  and  address  of  the  licensed  company  that  will  be
         disposing of the waste.  This will be issued to you on your site paperwork.

         5. Waste producer – where the waste has been produced by us, due to the removal of signage, we are the
         waste producer and Xmo Strata’s address should be entered here.

         Section B Description of the waste.

         1. The process giving rise to the waste, this would normally be “change of signage” or “painting site”.

         2. SIC code, this is the code for signage and painting works and will always be 45.34.

         3. Waste details, this is the details of the hazardous waste as detailed below:
              Description – a brief description of the waste, such as “fluorescent tubes” or “paint cans”.
              EWC code – this is the code number of the waste, examples are:
                 20-01-21   Fluorescent tubes.
                 20-01-27   Paint.
                 20-01-03   Fluorescent tubes mixed with general waste.
                 16-02-13   Fluorescent tubes mixed other electrical equipment (e.g. an entire light with lamps).
                 16-02-13   Sodium lamps.
              Quantity  –  this is the weight of the waste.  Where the waste is a liquid, the conversion of 1litre =
              1Kg can be used.
              Component  –  this  is the hazardous component of the waste, for fluorescent tubes this is Mercury,
              for paint it may  be Lead, this should be obtained from your COSHH sheets.
              Concentration – this is the amount of the component within the waste.  For fluorescent tubes this is
              0.01%, for other substances this will be identified on the COSHH sheets.
              Physical form – this will be the form of the substance – solid/liquid/gas/powder/sludge/mixed.
              Hazard code – this is the code stating the hazardous properties of the goods.  A copy of Appendix B
              showing the codes is attached.  For fluorescent tubes this will be H6 (toxic) and H14 (ecotoxic).
              Container  –  this is the quantity and size of the waste.  Examples are “5 boxes of 25” or “2no 5ltr
              tins”.
