The second section of section B3 of the report is for use when transporting “Dangerous for
Carriage” goods and is not normally relevant for our waste. The last column is the Special handling
requirements, enter any special requirements such as “fragile”.
Section C Carrier’s certificate.
This section must be completed by the company that will transport the waste. Where more than one
carrier is to be used, (for instance one carrier is returning the waste to a central location, and another to
the licensed disposal point) then a separate schedule of carriers must be completed. If you are
transporting the waste, then you must complete the section.
1. Carrier name – this is your name. On behalf of – this is Xmo Strata Ltd.
2. Carrier registration no – this is XXXXXXXXXXXX for Xmo Strata Ltd or BUS/723990 for Network
Cabling Ltd.
3. Vehicle registration no – give the registration number of your vehicle.
The section should be signed and dated by you.
Section D Consignor’s certificate.
This section should be completed by the consignor, the consignor is the person that generated the waste -
this will be us. You must enter your name, the company name, address, and sign and date where
indicated.
Section E Consignee’s certificate.
This section will be completed by the consignee, this is the person who received the waste (i.e. the
licensed waste company listed in section A4).
The consignee will confirm the details of the waste received, the quantity and how they are dealing with
the waste (Recovery / Disposal / Reject), the date, and vehicle registration number.
They will then sign and date the certificate and enter their waste management license number.

further official guidance is made available.
