                                                    Broken Wrist



           Please see enclosed a safety alert regarding a broken wrist that was shared with us by BP.

           This  incident  shows  the  need for all lifting operations to be  properly planned and the lift plan to be
           completely understood by all parties involved.

           Part of the planning process should be the need to segregate the work area and assess the movement of
           the load.  Anybody not directly involved with the lift should be removed from the segregated work area.
           Before any lift the lift plan should be discussed with all personnel involved to ensure they are clear on
           their role, including any actions in unintended  or emergency situations.

           Ensure any load is secured, if you  have any concerns about the load or the lift, stop and discuss your
           concerns with the lift supervisor and your line manager.




           We are all a product of learning. Every skill we have; everything we know; everything we believe and everything we do has
           been learned throughout the course of our lives. Some of us learn best by studying graphs and charts, some through visual
           presentations, and some by reading detailed text books. It is important that we receive information in a format that works for
