                                        Defective Equipment and Tools



           Please see enclosed a safety alert regarding an injury that was shared with us by BP.

           This  incident  again  reminds  us  of  the  need  to  continue  to  carry  out  last  minute  risk  assessments
           (LMRA)  throughout  the  day,  be  aware  of  our  surroundings  and looking for hazards.  Do not rely on
           weekly / monthly  audits to find problems with equipment  or tools, inspect them before you use them.

           If  you  find  equipment  or  tools  that  are  defective,  do  not  use  them,  contact  your  line  manage  for
           replacements.

           If you find a container or large item of plant on site that is hazardous, notify the site manager and raise a
           PI.  If its practicable for you to make the item safe on site, do so, if not try to isolate the hazard.

           When carrying out audits, take your time to look around and consider the potential hazards, no site is
           perfect so there will be hazards to find, document them and consider what action can be taken to resolve
           them.



           We are all a product of learning. Every skill we have; everything we know; everything we believe and everything we do has
           been learned throughout the course of our lives. Some of us learn best by studying graphs and charts, some through visual
           presentations, and some by reading detailed text books. It is important that we receive information in a format that works for
           us as individuals! That’s why at Xmo Strata we use a variety of communication platforms such as bulletins, newsletters,
           videos etc. However, if you feel there is a better way for us to share health and safety information, please let us know.
           Ultimately, your safety is in your hands – but we will do all we can to keep you well informed!
