                                              Electric Shock Injury



           Please see enclosed a self-explanatory bulletin that was shared with us by ExxonMobil.

           This  incident  is  a  reminder  of  the  need  to  stop  and  carry  out  a  separate  Last  Minute  Risk
           Assessment  (LMRA)  if  the  task  or  environment  changes.  When  working  on  existing
           infrastructure, it is not possible to always know exactly what you are going to find when you
           remove  a  panel  or  open an enclosure. Sometimes what you find is not at all what you were
           expecting.  It  is  important  in  these  cases  to  stop  and  re-evaluate  the  risk  of  the  job  before
           proceeding  -  you  may  even  find  the  risk  is  now  too  high  to  proceed  without  additional
           equipment or people.


           Remember to document on your clearance certificate any hazards identified as a result of any
           changes, as well as the additional control measures needed.
