Incident involving a slip on site
Please find attached summary from ABB Safety Bulletin relating to lessons learnt, relating to an incident on a
site in the UK. Please ensure that all lead engineers discuss with all on site and record as a toolbox talk.
The Xmo Strata and Xmo Veritas management team will follow up on their site visits and meetings with
employees and approved sub contractors.
ALERT DATA
During the replacement of a CCTV camera the employee tripped when walking towards the shop building to
check picture results on the office monitor. The incident happened on an Esso Service Station Collier Row in
Romford, Essex UK at 14:30 on the 31 Oct 2006.

INVESTIGATION RESULTS
The incident was investigated by ExxonMobil, ABB UK, ABB CT HSSE and the Contractor. The site was visited
and the area of the fall viewed and photographed. The employee involved was interviewed by ABB for the initial
16 point report and by his company management. A report was produced in both cases. The results of the
investigation recorded this was a RWI (Restricted Work Injury) as the employee continued to work on the
investigation, completed interviews, and completed an exercise relating to his companies business, and returned
to his normal duties on Monday the 6 Nov 2006.

WHAT HAPPENED?
After fitting a replacement CCTV camera in the car wash bay area and in order to check if images were being
received on the office monitor the employee walked towards the shop building and when stepping onto the
footpath he tripped on the kerb and fell. As he fell his shin and knee came in contact with the kerb and he
received cuts to the shin and knee.
