All of the above near misses could have caused injury or harm to a person or property but
were identified and dealt with, however, from ME downwards, we have failed to formally
record the root cause and action taken to prevent future incidents.

I fully accept my responsibility and will be addressing issues like those detailed above in a
different way from now on – may I please ask EVERY employee and sub contractor to do the
same.

I have attached our Hazard/Near Miss Procedure to this bulletin and an example of the near
miss form, copies of which are available from the office at any time.
