                                              Near Miss Reporting 3



           Following  the  2  previous  Safety  bulletins,  our  Near  Miss  Reporting  is  still  unacceptably  low,
           yet without too much difficulty I have managed to identify 17 specific examples of Near Misses
           that have happened on our sites and haven’t  been recorded as such.
           It is my intention to personally sit down with EVERY Xmo Strata and Xmo Veritas employee
           by the end of 2006 and explain face to face, the importance of reporting near misses. At this
           time, we will discuss near misses that have been reported or establish why none have.

           Specific examples (without allocating blame) that have been identified as having happened on
           our sites are as follows:-

           1.     Wheel bearings collapsed on scissor lift whilst being driven
           2.     Wind blowing scaffold boards loose
           3.     Unable to locate isolator resulting in being unsure if power isolated when working
                  on older sites with antiquated distribution boards
           4.     Driving company vehicle with bald tyres or lights not functioning correctly
           5.     Removing 5m long boxes without MEWPS  and not breaking the box down into
                  manageable sizes
           6.     Working off tower scaffold without fitting handrails or damaged components on
                  the unit
           7.     Scaffold company erected scaffold without toe boards
           8.     Driving company vehicles whilst using mobile phone
           9.     Turning blind eye to subcontractors working on site not wearing adequate PPE or
                  delivery drivers reversing trucks on site without aid of a banks-man
           10.    Items not detailed or missed from initial survey / validation: electrical cables
                  slung off wooden or steel power pylons,
           11.    Open skip sent to site and used when conditions dictate closed type skip required
           12.    Scissor lift or other mechanical plant spraying atomised diesel or hydraulic oil,
                  due to split hose
           13.    Using mobile phone on forecourt
           14.    Site operators not understanding English and therefore unable to understand or
                  explain emergency evacuation procedure
           15.    Barriers being moved by public / other contractors
           16.    A motorist jump starting a car next to the fuel dispenser
           17.    Another contractor filling up a plastic container with petrol and applying it with a
                  rag to clean the shop counter
