Health, Safety & Environmental Bulletin No 022
S Martin – 13 April 2006
Incident Reporting
In the past 2 months, 2 companies have been reprimanded by different customers of ours
because one of their employees suffered relatively minor injuries on site that weren’t reported
until several days later.
Following on from the 2 Near Miss Bulletins issued recently, it is very important that you ring
your Supervisor immediately if you suffer injury of any sort. Supervisors should make sure
that Russell Brown and the Project Coordinator are informed immediately. We are trying to
encourage an open culture where we can learn from mistakes, investigate root causes and
put procedures in place that will prevent someone else getting hurt – maybe more seriously
than you were.
We have a contractual obligation to advise BP/Bovis, Esso/ABB or Shell/JCI of any incident
where we are working immediately.
As a company, Xmo Strata are committed to achieving an Incident & Injury Free working
environment and we will only achieve this if we share both good and bad practice, analyse
what we can do better and share the learnings with each other. I am happy to share lessons
learnt with our customers and even our competitors, so that they can educate others to work
safely and improve safety standards across the entire industry.
Please continue to be alert, work safely, look for near misses and report anything that you
think could be done safer.
“let’s make sure that we continue to make our sites THE SAFEST by helping ourselves, our colleagues,
our customers and even our competitors by improving training, helping where necessary and
addressing any issues of concern there and then”
If you have any queries regarding the Safety Bulletin content or require further information please
contact any staff member of Xmo Strata Ltd.
