           Examples of Near Misses are as follows:-

               a)  A screwdriver being dropped from a scaffold tower.
               b)  A driver moving  safety fencing and driving  into the exclusion  zone.
               c)  A missing or badly fitting manhole  cover or interceptor grate that could cause a trip.
               d)  Other contractors who are not working safely (please talk to them about this at the time
                  and then report details of action taken as per your IIF (Incident & Injury Free) training  to
                  make the situation safer).
               e)  A site manager who cannot understand you enough  to be able to sign the clearance
                  certificate or know the emergency evacuation  procedure (if this is the case, you should
                  double check your procedures and work safely as if it was an unmanned  site but report
                  as a near miss).
               f)  Someone smoking on a petrol filling station.


           For clarification, if someone was not wearing a safety helmet on site, I would ask you to
           address this directly with them and advise your supervisor, making a note of it on your
           paperwork. We want to know about these sort of issues, but would not deem this as a “Near
           Miss” because that in itself could not cause harm or injury without another event happening
           (the guideline  is that common  sense should prevail).



           Our employee safety awards, presented in December 2005 recognised individuals  who had
           demonstrated their awareness of Health & Safety; thinking  about how to avoid near misses
           will reduce potential damage and accidents in the longer term and suggestions as to how we
           can do things safer or better will be well received when making the recommendations  for the
           2006 awards.
