Health, Safety & Environmental Bulletin No 215
M Mott – 4 November 2010
Lost Time Incident
Please see enclosed self explanatory bulletin from Johnson Controls.
This incident highlights the need to review all JSAs and identify any missing steps or
problems. This is not just “something they do at the office” – the most important step is
getting feedback from the people doing the job.
• Is the JSA practical?
• Are there any steps missing?
• Are there any hazards missing?
• Can the job be done safer?
• Are there any jobs you carry out that do not have appropriate JSAs?
You are legally obligated under section 14 of the The Management of Health and Safety
at Work Regulations 1999, to notify us of any shortcoming in our procedures. If you find
any issues with the JSAs or any other instruction – tell us about it!

About Risk
Risk management is a big step towards accident prevention; however, it is dependent on the risk firstly
being identified, then controlled. We need safety awareness to be instinctive and form an integral part
of both our working and home lives; it is this, in turn, that may save the lives of others. Remember: if
shared, risks can be mitigated before they pose a threat.
