Near miss incidents – Lessons Learnt

This week has seen 3 serious near miss incidents on our UK sites.
1. During the delivery of a large storage container to BP Downham the driver climbed
onto the container roof while it was still on the wagon. He had no fall protection with
a potential fall distance >5m. We have prohibited him from making further delivers to
our sites.
2. A shop fitter working at BP West End fell from a podium tower. He was pulling
himself along via cable trays and truncking attached to the ceiling. Fortunately there
was no serious injury, just minor bruising. Due to the individual’s honesty and
assistance during the investigation we have not requested his removal. His
employer has issued a warning that future disregard of site rules will result in
dismissal.
3. M&S advertisement banners were attached to the site fencing at BP Family Farm.
This restricted the line of sight for exiting traffic. While the Principal Contractor was
in the process of removing these there was nearly a collision between an HGV and
car. Site Managers must ensure these banners do not present a risk to others
including the public. High winds / restricted views need to be considered and if
banners create additional risk THEY MUST BE REMOVED ASAP. If in doubt speak to
your GA Project Manager or call me direct.
Unsafe attitudes and behaviours continue to be our biggest risk. We rely on trust and our
contractor partners choosing to work safely. But if lessons are not learnt, we will make
tough decisions where necessary.
THINK IIF AND DON’T BECOME ANOTHER ACCIDENT STATISTIC!
