Refrigeration Near Miss

A recent routine maintenance audit at Craigpark Connect discovered missing fan guards to refrigeration evaporator fans, which
had been removed by engineers on previous visits (top right). The fans continued to operate even after the main cabinet switch
was turned off. XXXX was contacted to send an engineer to site the same day. When the engineer arrived the situation allegedly
went from bad to worse. It is reported that the engineer
• Didn’t sign the site register
• Didn’t produce a PIA passport
• Didn’t complete a clearance cert and produce his method statement
• Didn’t isolate the cabinet and worked on it live
• Didn’t barricade and protect the area from customers and staff and exposed them to live electrical wiring (see top left)
The engineer has been prohibited from working on BP sites in future. XXXX is completing an urgent review of its procedures to
ensure only qualified staff with PIA passports and who demonstrate the correct behaviours are sent to work on our sites. We are
also investigating how the removal of the fan covers and the faulty isolation switch were not highlighted by previous crews.
