ACTION – RECOMMENDATIONS

1. Contractor to issue a safety flash to all their affiliates and sub contractors in Europe to help avoid a
repeat.
2. Contractor must produce a detailed JSA for this job.
3. Contractor employees must be re-rained in the use and benefits of LMRA
4. Timeliness of incident report is “immediate” within one hour ABB must be advised

5. Enter an emergency contact number on all new clearance certs so that several people have the
opportunity to make the call for assistance and to notify ABB/Esso of the incident.
6. Review the type of helmet worn and possibly introduce chin straps to ensure they stay on

Please ensure that by the end of this week this notification is cascaded to all of your relevant contractors and
employees. Please arrange for translation, hard copy distribution and/or verbal briefing where necessary.
