                                          Accident Triangle Statistics



           Our accident statistics for the first half of 2013 are now
           available, and show a significant improvement in hazard
           awareness  based  on  the  evidence  of  reporting  hazard
           spots/  potential incidents / near misses (PI/NM) as well
           as a drop in the number of first aid incidents.

           The  number of reported PI/NM’s has increased  43% on
           the same period last year, which is significant, but when
           taking  into  account  the  reduced  hours  worked,  the
           PI/NM’s per hour worked has increased by 118%.

           One  of  the  features  that  has  helped  improve  this year’s
           PI/NM  reporting is the use of IT tablets to send reports
           and  illustrative  photographs  electronically,  facilitating
           faster  delivery  of reports  to our customers  and no more
           paperwork lost in the post.

           Of  the  210 PI/NM’s  report this year, 70 (33%) of them
           were addressed and mitigated at source to eliminate the hazard and 21 positive interventions were made
           to prevent an unsafe act from occurring.

           It is widely believed that the public represent the biggest risk to us on a filling station, but interestingly,
           our statistics do not support this theory as the public are only responsible for 9% of the PI/NM’s.  The
           biggest problem identified is the site conditions not picked up on site safety tours, or the actions of the
           site staff; incidents as  a result of these represent 45% of all our PI/NM’s.  Incident examples involve
           blocked  fire  exits,  inappropriate  muster  points,  using  defective  equipment,  inadequate
           procedures/training, and lack of maintenance, e.g. pot holes, or other hazards easily identifiable from a
           simple site tour.

           Copies  of  all  investigations  are  sent  to the relevant customer so that they can investigate and where
           necessary  implement  corrective  actions.    We  therefore  encourage all crews to continue to report all
           NM/PI’s identified on site so that they can be addressed and the sites can be made safer environments
           for all patrons.
