MID Accident
Following the accident initially reported in Bulletins 354 and 364, which are on our website, or

http://www.xmostrata.com/health/2013-06-27.php), please see attached safety alert with more detailed
information and root cause analysis’ that was issued by ExxonMobil.
As previously noted, this incident highlights the need for strict controls whilst carrying out crane
operations, and the use of trained, competent staff. All our employees that carry out lifting operations
are NPORS qualified and have comprehensive safety documentation and safe systems of work to
ensure the lifting operation is carried out in the safest possible manner; this alone, however, does not
guarantee that an accident won’t happen.
LMRA’s, disciplined controls and adherence to your training and safe systems of work are critical in
preventing incidents of this type.
If for any reason you are not able to follow the documented safe system of work, you must stop and
carry out a formal LMRA (and document the changes) and if the change is significant or you are
unclear as to how to ensure the process is safe, please discuss and agree the course of action with your
Line Manager before proceeding.
If you feel it’s not safe, don’t work!
