Hand Injury from Angle Grinder

This incident is another example showing the importance of the Last Minute Risk Assessment
(LMRA). Even if the method statement / JSA /JHA is fully complied with there may be site specific
conditions that need attention.
Site conditions can change during the works, and regular LMRA's should be carried out to ensure that
the new conditions have not introduced new hazards.
Our safe systems of work document the best practices for carrying out the works, and must be complied
with (unless site conditions require more stringent controls which should be documented on Clearance
Certificates). If you feel that a different method would be better, or have any feedback on the JSA's,
please contact your line manager – we want to learn and improve our processes from your feedback.
We are all a product of learning. Every skill we have; everything we know; everything we believe and everything
we do has been learned throughout the course of our lives. Some of us learn best by studying graphs and
charts, some through visual presentations, and some by reading detailed text books. It is important that we
