                                 Incident With Hammer - Broken Fingers



           Please see enclosed self-explanatory bulletin from JCI.

           This  incident  shows  the  importance  of  always carrying out a Last Minute Risk Assessment  (LMRA)
           before carrying out any work.  This is especially true if you find the work has to deviate from the safe
           system of work detailed on the JHA/JSA or clearance certificate due to unforeseen  circumstances.

           The  LMRA  process  should consider all the hazards; please  use your LMRA card as a reminder and
           remember to document any changes  to your working procedures on your Clearance Certificate.





           We are all a product of learning. Every skill we have; everything we know; everything we believe and everything
           we do has been learned throughout the course of our lives. Some of us learn best by studying graphs and
           charts, some through visual presentations, and some by reading detailed text books. It is important that we
