Rigging Failure
Please see enclosed a safety alert regarding a rigging failure that was shared with us by ExxonMobil.
When planning jobs, every effort should be made to make the job as safe as possible and to ensure all
equipment is in good working condition, however the case where something unexpected happens should
also be planned. This is why we have safety buffer zones around the work area, why we use vans as
barriers, why we don’t allow lifting operations to happen above areas where people are working.
When reviewing the JSA and documenting your LMRA, always ask yourself what could go wrong, and
how you can eliminate/reduce the chance of that happening or of the consequences if it does.
Remember to continue to re-assess the risks on site throughout the job, the nature of our work means the
conditions on sites we work on change often, particularly due to traffic entering and leaving site
regularly. Regularly re-assess the situation and where necessary place additional controls. In some
circumstances it may even be necessary to stop work due to significant changes. Be sure to document
the changes and controls on your clearance certificate.

been learned throughout the course of our lives. Some of us learn best by studying graphs and charts, some through visual
presentations, and some by reading detailed text books. It is important that we receive information in a format that works for
us as individuals! That’s why at Xmo Strata we use a variety of communication platforms such as bulletins, newsletters,
videos etc. However, if you feel there is a better way for us to share health and safety information, please let us know.
