                                             Cooling Tower Fatality



           Please see enclosed a safety alert regarding a facility that was shared with us by Quant  Service.

           This tragic incident is a reminder of the need to consider all risk categories when carrying out LMRA.
           Just  because you are working at ground level don’t mean there’s no need to consider falls, any open
           chamber, excavation, change in level, starcase, etc should be considered and controlled.

           Control  measures  should  also be considered; in this case we see how completely ineffective  ‘hazard
           warning’ tape was as a physical barrier.  If you are controlling access to a fall hazard, ensure the barriers
           are suitable and sufficient  for the task.

           If you don’t have  suitable or sufficient barriers to adequately protect the hazard, stop and contact your
           line manager.



           We are all a product of learning. Every skill we have; everything we know; everything we believe and everything we do has
           been learned throughout the course of our lives. Some of us learn best by studying graphs and charts, some through visual
           presentations, and some by reading detailed text books. It is important that we receive information in a format that works for
