Action points:-

1) All crews to receive a HSSE bulletin and toolbox talk on this incident stressing the need to assess
loads and follow the same system of work documented.

2) A review of delivery vehicles to be undertaken to establish if a flat-bed lorry can be sourced so
we can crane the crate off the vehicle to reduce manual handling.
