Safety Incident Lessons Learnt
Please see enclosed lessons learnt report regarding recent safety incidents that were who kindly shared
with us by BP.
These incidents show the importance of following the documented safe system of work
(JSA/JHA/Method Statement) and carrying our Last Minute Risk Assessments (LMRA).
Regularly carrying out high risk work can cause people to because accustomed to the risks and start
cutting corners or becoming “asleep at the wheel”. The JSA’s document steps to be taken to ensure
your safety, so missing steps or controls reduces your protection and can result in accidents.
There will always be site specific risks on site that were not documented on the JSA, therefore it is
important to always carry out a LMRA to identify the specific hazards on the site using the six hazard
categories covered in your training, and continue to do so thoughout the day as activities and site
conditions change.
We are all a product of learning. Every skill we have; everything we know; everything we believe and everything we do has
been learned throughout the course of our lives. Some of us learn best by studying graphs and charts, some through visual
presentations, and some by reading detailed text books. It is important that we receive information in a format that works for
us as individuals! That’s why at Xmo Strata we use a variety of communication platforms such as bulletins, newsletters,
videos etc. However, if you feel there is a better way for us to share health and safety information, please let us know.
Ultimately, your safety is in your hands – but we will do all we can to keep you well informed!
