                                               Safety Observations




           At Xmo Strata, our safety culture  is  based on a simple principle  –  if you spot a potential hazard and
           make it safe, people won’t be hurt and your action could prevent an injury.

           In 2013/2014 – the HSE reported the following:-

           4.7 million days were lost to workplace injury

           629,000 injuries occurred at work, of which 148,000 led to over-7-days absence

           133 workers were killed at work, of which 42 were in the construction industry


           Our Leading Indicators - 2014

           Leading indicators are positive actions that we take to prevent people from being hurt.

           The  safety  triangle  that  we  share  with  all  employees  each  month  as  part  of  The  HSSE  Lead  Team
           meeting minutes shows that we identified 324 potential hazards (also known as Potential Incidents or
           safety Observations), equating to one for every 178 hours worked on site by us.

           Of the 324 hazards that we spotted, we made 143 of them safe at the time of discovery (41%), equating
           to one every 403 hours worked and we positively intervened to prevent someone from doing something
           unsafe, or that could hurt someone on 27 occasions.

           These  leading  indicators  are  very  positive  and  credit  must  go  to  our  clients  that  encourage  us,
           incentivise us and recognise safety best practice, but more credit should be given to our employees who
           take  the  time  to  identify  hazards,  proactively  make  them  safe,  make  positive  interventions  when
           necessary and take the time to report them to us.


           Our Lagging Indicators - 2014

           Lagging indicators are negative issues (that we really want to know about) where an incident occurred.
           In most cases, the incident could have been prevented if someone had spotted or foreseen the hazard –
           we had 3 First Aid incidents.
