BP Site Incident
We have recently been notified of the following incident that occurred on a BP site. Although
the actual work being carried out is not relevant to our works, the cause of the accident, i.e.
working at heights, is very relevant. It is important that all works carried out at heights are
appropriately risk assessed, using the clearance certificate and the work at height form.
From Global Alliance Bulletin:
Unfortunately we have had a Day
Away From Work Case (lost time
accident) this month. As shown in the
re-enactment photo, a refrigeration
engineer was replacing a ceiling tile
at BP Queensway North and slipped
while descending the ladder. This
resulted in a fracture to his right
wrist. Although he is expected to
make a full recovery, the
investigation has highlighted some
major concerns that must be
addressed.
Findings:
• The contractor used the site stepladder for access, which was not suitable.
• The method statement and clearance cert did not contain enough detail on the means of access
and ladder use.
• The location of the cold room probe (which had to be replaced) resulted in access through the
false ceiling onto the cold room roof.
• Late accident reporting – the contractor completed the site accident book and notified his
supervisor BUT due to key personnel being on holiday the supervisor didn’t notify anyone else in
his company or the Global Alliance.
Corrective Actions:
1. BP Site stepladders ARE NOT to be used by contractors under any circumstances, unless there is
an absolute emergency.
2. Ladders must be fit for purpose and for access only. If work areas as so small steps must be used,
method statements must make this clear with specific details explained on the clearance certs and
agreed with the BP Site Manager.
