Please see enclosed self explanatory JCI safety bulletin.
It is important to re-assess the risks when any factor changes on site, including
unexpected difficulty in carrying out the task or changes to the sequence of works. A
last minute risk assessment must be carried out and the clearance certificate updated.
Any fault finding carried out on live equipment must be assessed as a separate task -
don't let yourself get so focused on the fault that you miss the big picture - stop and plan
how you are going to carry out the fault-finding, and document the steps on the
clearance certificate.
Risk management is a big step towards accident prevention; however, it is dependent on the risk firstly
being identified, then controlled. We need safety awareness to be instinctive and form an integral part
of both our working and home lives; it is this, in turn, that may save the lives of others. Remember: if
shared, risks can be mitigated before they pose a threat.
