Hazard Spot and Near Miss Reporting
How are we doing?
I am delighted to confirm that in the first quarter of 2013, we achieved 100% participation from site
teams in our hazard spot/near miss reporting initiative and would like to congratulate and thank you all.
So why is that important?
Many of you will have seen that a police officer attempted to sue a forecourt garage owner because she
tripped on a kerb whilst on a 999 call. Please hold control button and click on the link below for more

This highlights the fact that taking the time to identify a potential hazard and addressing it at source to
eliminate the risk does add real value to our customers in terms of protecting them from litigation and
just as importantly, the time taken to defend themselves if they are sued. When we introduced the
concept of hazard identification and mitigation, we were identifying 1 hazard for every 3500 man hours
worked, in the first quarter of this year, we identified 1 hazard for every 100 man hours worked. Since
we started recording this data on our XmoMan Management Information System, our site teams have
addressed and made safe more than 590 potential hazards and made more than 240 positive
interventions (to stop or prevent dangerous activities).
The graph below shows the hazards identified per 100,000 hours worked.

We are all a product of learning. Every skill we have; everything we know; everything we believe and everything
we do has been learned throughout the course of our lives. Some of us learn best by studying graphs and
charts, some through visual presentations, and some by reading detailed text books. It is important that we
receive information in a format that works for us as individuals! That’s why at Xmo Strata we use a variety of
communication platforms such as bulletins, newsletters, videos etc. However, if you feel there is a better way for
us to share health and safety information, please let us know. Ultimately, your safety is in your hands – but we
will do all we can to keep you well informed!
