Health, Safety & Environmental Bulletin No 351

                                                    11 April 2013

                                              LOTO LSR Violation

Please see enclosed self-explanatory bulletin that was shared with us by JCI.

It is not unusual to find the labelling of circuits to be poor, and can sometimes be difficult to identify
which breakers need to be operated to correctly isolate the circuits you need to work on. It is therefore
imperative that you check the power at the sign before touching any electrical components to ensure it
           is dead.

All crews have recently been issued with new probe testers and proving units, and have been trained on
the correct use of these devices to ensure crews are able to prove circuits are dead (see bulletin 249).

If you are unable to identify the breaker to isolate, contact your line manager – do not work until you
have proved the circuit is safely isolated and locked off.

The routine decisions we make on a daily basis are often more as a force of habit than something we spend time
thinking about – looking before we cross the road, moving out of the way of falling objects etc. But what about
decisions that are just as important as these that we may think about, but choose to ignore – like seeing
someone doing something unsafe, or not eliminating a simple hazard when we spot it? These decisions are just
as important, and we want these to become just as instinctive as crossing the road. Please visit YouTube and
type ‘Xmo Strata reverse thinking’ into the search box – let’s make each decision count this year!
