Lessons learned

               •  The value of ‘safety walks’ – if this had not been spotted, members of the public or site
                   visitors/staff could have been injured
               •  Even professionals can have accidents if a proper LMRA isn’t carried out – they should they
                   have used a platform or cherry picker instead of their ladder
               •  The fire brigade should have established a safe area below the canopy in anticipation of falling
                   debris
               •  When loose paneling is spotted, the area directly below should be isolated/secured to prevent
                   injury in the case of falling debris
               •  If you ever have to speak to the emergency  services, make them aware of the potential dangers
                   and highlight  this incident as an example



           We are all a product of learning. Every skill we have; everything we know; everything we believe and everything
           we do has been learned throughout the course of our lives. Some of us learn best by studying graphs and
           charts, some through visual presentations, and some by reading detailed text books. It is important that we
