Behavioural Safety: Breach of Life Saving Rules
Recently, an Xmo Strata subcontractor was witnessed breaching one of Xmo Strata’s site rules and
Shell’s Life Saving Rules on a Commission Operators site.
He always leaves his phone in the van as instructed. The van was parked on site.
He went to his van at a convenient pause in the work schedule for that day to make a call to reserve his
accommodation for the night. Whilst on the call, another Xmo Strata Operative tapped on the window
to state that he would need to move his van to allow a delivery driver access to an area of the site.
Instead of hanging up the phone before turning the ignition, he remained on the call whilst moving the
van. This was witnessed by the Artelia Project Manager and the Shell Commissioners.
They immediately escalated the incident within both organisations classifying it as a breach of one of
the Shell Life Saving Rules:
How many of you might have made the same mistake? How many would argue this is an easy mistake
to make? Would you be saying the same thing if he’d just reversed and hit your child?
