Root cause of the incident:
• Failure to carry out a last minute risk assessment before starting a new task.
• Distractions of phone calls and a desire to ensure the clients requests were carried out led to the
engineers failing to complete a new clearance certificate.
Lessons learnt:
• Last Minute Risk Assessments must always be completed for any new task.
• No-one should feel pressured to get work done quickly. If there is any concern, staff should call
the office, explain the situation and follow appropriate safety procedures.
I would like to thank both employees for allowing us to use this bulletin for others to learn from and
would like to take this opportunity to stress that it is Xmo Strata’s policy that harnesses should be worn
and secured to a suitable anchor point in ALL MEWP’s.
Risk management is a big step towards accident prevention; however, it is dependent on the risk firstly
being identified, then controlled. We need safety awareness to be instinctive and form an integral part
of both our working and home lives; it is this, in turn, that may save the lives of others. Remember: if
shared, risks can be mitigated before they pose a threat.
