Lessons Learnt – The importance of LMRA’s
The following incident is shared with you to highlight the importance of carrying out Last Minute Risk
Assessments (LMRA).
Investigations have now been conducted into the incident and results have been shared with the staff
involved and the customer. Thankfully no-one was injured. But a serious lesson has been learnt.
This incident does involve Xmo Strata employees and is shared according to our policy of being open
and honest about all health and safety issues – including our own failings. Hopefully this will serve as a
reminder to all those working in hazardous environments – LMRAs are vital to site safety.
The incident:

Late in the afternoon of the 27 January it was reported that two Xmo Strata engineers had been
working on a site in a MEWP without wearing safety harnesses.
An urgent investigation was called immediately conducted.
The two Xmo Strata engineers were informed that they would be suspended on full pay pending the
outcome of the investigation.
The customer was met on site on Friday 28 January and he explained what had happened and who had

seen the incident. It was agreed that statements would be taken from three witnesses plus statements
from the customer. Photos were also taken of the shop fascia they were working on and the machine
involved.
A letter was then sent to the engineers and customer involved to confirm suspension of the Xmo Strata

employees and requesting attendance at investigation meeting on Monday 31 January.
