Health, Safety & Environmental Bulletin No 224

                                         S Martin – 23 December 2010

                                                      Fatal Fall

Please see enclosed self explanatory incident report from Johnson Controls and guidance
on working in the dark from Gilbarco.

This incident highlights the reasons why suitable barriers are imperative around work
areas, especially at night or when the site is unmanned. It also highlights the need to
ensure you have adequate illumination to carry out all works - if you are struggling to
see, stop and arrange additional illumination.

                                                                                          Share Thoughts
                                                                                            About Risk

Risk management is a big step towards accident prevention; however, it is dependent on the risk firstly
being identified, then controlled. We need safety awareness to be instinctive and form an integral part
of both our working and home lives; it is this, in turn, that may save the lives of others. Remember: if
shared, risks can be mitigated before they pose a threat.
