Even safety equipment can be unsafe!
Please see below 2 self explanatory bulletins issued by London Underground and Network Rail
respectively.
In both cases, a logical assumption may very well be that “the equipment must be ok because it’s
made by a reputable company” and the harness in particular is specially made to protect you.
At Xmo Strata, we have anecdotal evidence of new, faulty equipment and as a result of that have a
policy of PAT testing electrical equipment (for example) even though it is new.
Always remember that even the best manufacturers can make mistakes, the most reliable machines
can develop faults and the most vigilant inspector can fail to spot a flaw.
As discussed in depth at our Hearts & Minds behavioural safety workshops, always use your own
senses (look, listen, feel and smell) to carry out your own inspection of your plant, tools, equipment
and PPE before starting work or putting your trust in them; don’t assume they’ll be ok!
