As part of our ongoing Health and Safety incident reviews, we have noted that the number of electrical
related near-misses had more than doubled, increasing from 6 near misses in 2007 to 13 in 2008.
Although most of these are reports of near-misses unrelated to Xmo work, they highlight the significant
risk that electricity poses to all contractors and sometimes the general public on site; electrical incidents
represent 14% of all the near-misses reported to us.
In order to highlight this risk and ensure control measures are applied before we carry out our work, i.e.
Log-out Tag-out, we have produced a checklist highlighting the required steps necessary to ensure the
safe isolation of the energy source.
These checklists, supported by incident footage and a training DVD were discussed in depth at our
December Safety Awareness Day and distributed to all attendees. A copy is attached to this bulletin.
These checklists must be utilised when you are going to work on any equipment, signage or tools that
are powered by electricity or other energy source (e.g. pneumatics, stored kinetic energy etc). Use the
checklist to ensure you follow the best practice for isolation and record the risk and control measures
taken on your clearance certificate.

Being safe depends on the choices we make everyday. The right choices can save lives. We want to
share safety information and help to raise industry awareness. Safety is not about getting one over on
the competition. We need to talk openly, share what we know and make the right choices.
