                                           Global Alliance Accidents

           The last few days have seen an accident and a serious incident occur on our UK sites.

           1.  An engineer fell over while exiting the site office at BP Harlow CS. He didn’t notice a small kerb,
              lost his footing suffering bruising to his leg and was assigned light duties the next day. While every
              effort  is  made  to  identify  and  control  site  hazards,  it  is  important  to  pay  attention  to  your
              surroundings  and  footing  at  all  times  ESPEICIALLY  when  you  arrive  on  a  new  site  and  /  or
              conditions change. If you notice a potential hazard, inform the Site Manager ASAP.

           2.  During the offloading of large type Brooklyn chiller units the forklift forks went too far and slightly
              lifted  the  unit  behind  causing  it  to  fall  from  the  delivery  vehicle.  No  one  was  hurt  due  to  the
              exclusion  area  in  place  at  the  time.  The following amendments MUST be made to the offloading
              method statement  with immediate effect:

              •  Only one side of the delivery vehicle is to be uncovered during unloading.
              •  The banksman must undertake a visual check that the forks are correctly in place and confirm to
                 the driver BEFORE each unit is lifted.
              •  The banksman must be in a safe position while the forklift is positioning, during the lift and must
                 not put himself at risk at any time.
              •  The positions of the driver and banksman MUST be sketched on the method statement to ensure
                 visual contact is maintained at all times.
              •  All existing requirements for exclusion zones remain in place.
