Fatalities
This week we have been notified of three fatalities that have occurred on fuel station forecourts.
In the USA a father and son were working in a trench when it collapsed. The father died at the scene and
the son is recovering in hospital. On the same day, a scaffolder fell approx 10m to his death while
striking scaffold.
In South America an electrician fell to his death whilst working within a canopy. He had entered the
canopy structure to carry out maintenance on an electrical box within the canopy. He was wearing a
harness and had connected the lanyard to the canopy structure, but had not secured the lanyard to his
harness. Whilst working from a plank he had taken with him it is believed the plank slipped off the
support beams and he fell through the lower deck of the canopy.
All our thoughts are with the families and friends they left behind.
This month we have also had an accident occur on site with the injured party suffering a suspected
hairline fracture of the foot. This has developed into a reportable accident and a detailed report has been
sent to the customer and the HSE. The injured party was trapped between a Heras fence base and a
MEWP when the vehicle was maneuvering. Both parties were trained to UK PIA, IPAF, PASMA and
CSCS but were not paying enough attention to the potential danger. The driver was in a position that he
could not see the injured party, and there was no banksman coordinating the maneuver. PLEASE NOTE
THAT A BANKSMAN MUST BE VISIBLE TO THE DRIVER WHEN ANY PLANT IS BEING
MOVED.
As can be seen, no matter how hard we try to stress safety, it is your actions on site that make the
difference between a happy, safe site and a tragedy. We provide and insist that you wear protective
footwear, headwear, eyewear, high viz vests, carry out regular toolbox talks highlighting the hazards on
site and all of you have attended many safety training courses including most recently CSCS. We urge
you to think of the three recent deaths and the injury to your colleague and remember those when
working on site.
