Lock Out Tag Out Training

This incident referred to in the bulletin highlights the need to ensure all employees are trained in the
use of the equipment they use, particularly equipment that is designed to ensure the people’s safety.

We have recently issued new probe testers and proving units to all site teams, and to ensure they are
aware of the test procedure comprehensive training sessions have been carried out. The training
content included one-on-one training sessions on a specially constructed training bed, where scenarios
were discussed, and participants were able to ask questions, engage in the training, practice using the
testers and demonstrate their understanding of the procedure.

The routine decisions we make on a daily basis are often more as a force of habit than something we spend time
thinking about – looking before we cross the road, moving out of the way of falling objects etc. But what about
decisions that are just as important as these that we may think about, but choose to ignore – like seeing
someone doing something unsafe, or not eliminating a simple hazard when we spot it? These decisions are just
