                                                      Eye Injury



           Please see enclosed self explanatory HSE alert received from ExxonMobil.

           This incident shows the importance of the Last Minute Risk Assessment (LMRA), even
           if  the  method  statement  /  JSA  is  fully  complied  with  there  may  be  site  specific
           conditions that need attention.

           Site conditions can change during the works, and regular LMRA's should be carried out
           to ensure that the new conditions have not introduced new hazards.

           Our JSA's document the best practices for carrying out the works, and must be complied

           with (unless site conditions require more stringent controls which should be documented
           on Clearance Certificates).  If you feel that a different method would be better, or have
           any feedback on the JSA's, contact your supervisor.






           Risk management is a big step towards accident prevention; however, it is dependent on the risk firstly
           being identified, then controlled. We need safety awareness to be instinctive and form an integral part
           of both our working and home lives; it is this, in turn, that may save the lives of others. Remember: if
           shared, risks can be mitigated before they pose a threat.
