                                                Breaker Near Miss



           Please see enclosed a safety bulletin that was shared with us by BP.

           This incident shows the importance of carrying out daily and before-use checks of plant and equipment.
           Before using any plant or equipment complete an inspection of it, use the relevant safety check sheet to
           guide you as to what to check, but don’t blindly follow the checklist, this is not a tickbox exercise – take
           a step back and look at the equipment, does it look in good condition and is it fit for the purpose you are
           using it for?

           Completing  the  paperwork  checklist  is  important,  but  not  as  important  as  actually  checking  the
           equipment!  The forms and checklists are a guide and reminder of the main areas that need checking,
           but  they  cannot list every possible item that should be looked at  –  use your common sense and think
           what could go wrong with this specific item of plant?

           If you find plant or equipment  that is faulty, do not use it and contact your line manager.



           We are all a product of learning. Every skill we have; everything we know; everything we believe and everything we do has
           been learned throughout the course of our lives. Some of us learn best by studying graphs and charts, some through visual
           presentations, and some by reading detailed text books. It is important that we receive information in a format that works for
           us as individuals! That’s why at Xmo Strata we use a variety of communication platforms such as bulletins, newsletters,
           videos etc. However, if you feel there is a better way for us to share health and safety information, please let us know.
           Ultimately, your safety is in your hands – but we will do all we can to keep you well informed!
