Poorly Wired MID
Please see enclosed safety alert regarding a poorly wired MID shared with us by BP.
Unfortunately, poorly wired circuits on site are common, and this is the second most common category
of potential incident reports we receive. We have recorded 370 electrical PIs related to poor electrical
condition, 20 PI’s just this year. In 36 of those PI’s this resulted in the equipment having no earth, and
24 resulted in exposed live parts.
Before working on any equipment always isolate the energy source (LOTO), and test the circuit to
ensure it is dead. However beware that circuits are regularly min-labelled and automatic switching can
mean circuits are switched on without warning, this can result in you inadvertently isolating the wrong
circuit and testing a circuit that is only dead due to the light levels being good. Always test a cable you
think is dead immediately before touching it. If possible, avoid touching any exposed conductor.
If you see any poorly installed electrical equipment, always report it – you may be saving somebodies
life.
If you are in doubt as to which circuit needs to be isolated, contact your line manager so an electrician
can investigate.
