Pile Driving Boom Failure

Please see enclosed bulletin regarding equipment failure, shared with us by Quant.

This incident is a reminder to always check plant and equipment before use, and never to disable or
remove safety equipment. Never modify equipment in ways not intended by the manufacturer. If the
equipment is not fit for purpose, contact your line manager.

Before using any equipment, inspect it, using checklists for specific items where applicable. If you find
any faults or missing parts report them to your line manager – do not use faulty or damaged equipment.

All plant and equipment used on site must be used in accordance with the manufacturers instructions.

Read the instructions before using any new plant, and contact your line manager if there is anything you
do not understand, or if you cannot find the instructions.

We are all a product of learning. Every skill we have; everything we know; everything we believe and everything we do has
been learned throughout the course of our lives. Some of us learn best by studying graphs and charts, some through visual
presentations, and some by reading detailed text books. It is important that we receive information in a format that works for
