Health, Safety & Environmental Bulletin No 476

                                                   13 August 2015

                                                     Trip Injury

Please see enclosed safety alert regarding a trip injury that was shared with us by BP.

According to HSE statistics, slips, trips and falls on same level are one of the most common causes of
accident, with 13841 seven-day accidents, 7742 major accidents and 2 fatalities during 2013/2014.

When you are carrying out your LMRA (Last Minute Risk Assessment), always beware of trip hazards,
they tend to be very common and can easily be overlooked. Once you have identified a trip hazard,
consider how you can control it, can you remove the hazard or segregate it (barrier or cover the area)?
Make sure everybody on site is aware of the hazard and that you have documented how you are
           controlling it.

We are all a product of learning. Every skill we have; everything we know; everything we believe and everything we do has
been learned throughout the course of our lives. Some of us learn best by studying graphs and charts, some through visual
presentations, and some by reading detailed text books. It is important that we receive information in a format that works for
us as individuals! That’s why at Xmo Strata we use a variety of communication platforms such as bulletins, newsletters,
videos etc. However, if you feel there is a better way for us to share health and safety information, please let us know.
Ultimately, your safety is in your hands – but we will do all we can to keep you well informed!
