Health, Safety & Environmental Bulletin No 469

                                                    25 June 2015

                                                      Flash Fire

Please see enclosed safety alert that Jacobs have kindly shared regarding a flash fire.

This incident shows the importance of following the documented safe system of work
(JSA/JHA/Method Statement). Regularly carrying out high risk work can cause people to become
accustomed to the risks and start cutting corners or becoming “asleep at the wheel”. Safe systems of
work document steps to be taken to ensure your safety, so missing steps or controls may reduce your
protection and can result in accidents.

It is also necessary for you to report if you consider the JSA to be incomplete, inadequate or
impractical. If you think the JSA is not fit for purpose, it is more likely that you (or others) will not
follow it and that may have potentially serious consequences. Please always tell your line manager if
you have concerns, or feel that s JSA/JHA or SMS can be improved.

We are all a product of learning. Every skill we have; everything we know; everything we believe and everything we do has
been learned throughout the course of our lives. Some of us learn best by studying graphs and charts, some through visual
presentations, and some by reading detailed text books. It is important that we receive information in a format that works for
us as individuals! That’s why at Xmo Strata we use a variety of communication platforms such as bulletins, newsletters,
videos etc. However, if you feel there is a better way for us to share health and safety information, please let us know.
Ultimately, your safety is in your hands – but we will do all we can to keep you well informed!
