                                      Recent incidents – Lessons Learnt



           Please  see  enclosed  self explanatory safety  alert and bulletin received from  the Global
           Alliance.

           Lack of concentration is a factor in a significant number of accidents and this can take
           on many forms, distractions, over-confidence, tiredness or just lack of fore-thought.

           It is important to stop before you carry out any task (even tasks you carry out every day)
           and  consider  the  risk  involved  and  any  control  measures  needed.  Last  Minute  Risk
           Assessments (LMRA’s are an essential part of being safe at all times, whether on site, en
           route, or at home.


           If you are distracted for any reason, stop and re-evaluate the situation before continuing -
           it is very easy to miss a step or repeat a step after being distracted - check first.


           The following pages contain  details of incidents not relating to Xmo Strata, but caused
           by  the  injured  party  not  stopping  and  considering  the  potential  consequences of their
           actions.  The injured parties were lucky not to suffer more serious injuries.






           Risk management is a big step towards accident prevention; however, it is dependent on the risk firstly
           being identified, then controlled. We need safety awareness to be instinctive and form an integral part
           of both our working and home lives; it is this, in turn, that may save the lives of others. Remember: if
           shared, risks can be mitigated before they pose a threat.
